About Your Visit

Thank you for allowing Cardiology Consultants of Atlanta to participate in your cardiovascular care. Our experienced staff is committed to providing comprehensive quality cardiovascular care in a personable, professional and compassionate manner. In order to better serve you and make your visit productive, we ask that you bring the follow to your office visits:

  • Current list of medications
  • Insurance Card and photo ID
  • Specialist Referral from primary care physician (if your insurance plan requires)
  • Pertinent laboratory, previous diagnostic testing or other information you feel may be beneficial to your overall care
  • Co-Payment or other monies due at the time of service (as outlined by your designated insurance plan)

Insurance

Your insurance is a unique contract between you, your employer, and your insurance company. It is important for you to understand your insurance plan because not all services are covered by all insurance plans (for example, a routine physical exam is not covered under Medicare or some PPO plans.) However, this is not to be confused with the physician’s determination of which services are medically necessary or appropriate.
Co-payments and payments toward your deductible must be paid at the time of service. If you are having problems meeting your financial obligations please notify our management staff so that we can better assist you through our unique payment options.

Missed Appointment Fee for Office and Hospital Based Diagnostic Tests

We set aside valuable lab equipment, technician, and physician time when scheduling diagnostic testing. Too often, an appointment is not kept, or is cancelled with too little notice to schedule with another patient. Therefore, our policy now requires a fee for any office or hospital based diagnostic test as well as office consultation that is not kept, or cancelled or rescheduled with less than 24 hours notice (only business days included).


Medical Records

A nominal fee will be charged to cover our costs when we provide a copy of your records to you or third party other than another physician.


Prescription Refill Policy

Medication errors are one of the most common errors in the practice of medicine. These errors primarily arise from the lack of involvement of patients to take the necessary time to know the medications they take or write them down and bring them to each clinic visit. Medication errors not only pose dangerous risks to patients but also expose medical practices to liability. To prevent medical errors, we ask all our patients to bring your medications (or a written list) to the clinic at each visit. We prefer to address all prescription refill requests directly through our office at the time of your visit or rarely via the prescription refill line. We do not interact with commercial pharmacies via telephone or automated faxes.

The purpose of the prescription refill line is for the rare occasion a prescription is lost, appointments are missed / rescheduled or you forget to address your requests at the time of your visit. It is not to be abused by calling excessively as many patients take multiple medications. We ask that you take the time to look on the bottles, see the remaining refills and request them all at the time of your office visit or when you call the prescription line have all the meds that you are requesting refills for to last you until your follow up visit. Due to the abuse of this courtesy service (that we are not reimbursed for), we must limit the number of phone refill requests to 3 per year thereafter you will need to schedule an office visit specifically to address your prescriptions.

  • In order to prevent confusion and medical errors:
  • We prefer to address all prescription refill requests at the time of your office visit.
  • Verify your prescriptions with regards to dosages and refills prior to leaving the pharmacy.

We ask that you bring your medications or a list of your medications with refill request to each office visit. For your convenience, we have provided prescription cards for you to list your medications and place them in your purse or wallet.

Please allow 48-72 hours for all prescription refill requests to be completed. We allow 3 prescription refill requests outside of your routine office visit per year; thereafter, you will need to schedule an appointment to address this ongoing problem.
